At Fort Tryon Center, our long term residents benefit from rehabilitative, restorative, and continuous skilled nursing care. Programs are designed to medically stabilize, strengthen and support residents both physically and emotionally. Our clinical care team is fully equipped to manage complex medical care and rehabilitation associated with multiple acuity healthcare needs and chronic conditions often associated with older populations.

With special attention to maintaining high functioning status, we have pioneered many unique programs that assist residents in being as independent as possible.
Our activities program offers an array of programs that focus on individual hobbies and interests. Activities are designed with recreational and therapeutic emphasis to maintain physical, cognitive, social, creative, spiritual and intellectual abilities. Our wheelchair-accessible vans enable residents to take frequent off campus trips.
Alzheimer’s/ Dementia Care
Dealing with a loved one that has been affected by Alzheimer’s can be a challenge. Fort Tryon Center specializes in the care of residents with Alzheimer’s disease and related dementia. Our environment puts families at ease knowing their loved ones are in a safe place. Our compassionate staff are not only licensed and trained to care for residents with dementia, but also emphasize respect and dignity in caring for them.
Our nursing home serves residents who require assistance with activities of daily living. The team reviews resident’s physical and behavioral issues as well as life history in creating daily routines and care plans for our Alzheimer’s/ Dementia population. Our facilities are designed to provide familiar surroundings that offer a reassuring sense of safety and security.
Activities and daily living chores at Fort Tryon Center add meaning and routine to our residents’ days and encourage the use of their remaining skills. Residents are assisted with specific activities that help stimulate memory and build friendships among the staff and other residents.
Fort Tryon Center is equipped with video cameras on all floors and has the availability of a wander alert system which sounds if a confused resident wanders away from the living areas. These systems help our staff maintain the safety of those with dementia.
